As a convenience to our patients with dental insurance, our office submits the charges to your insurance carrier at no extra cost to you.
Please understand that your insurance is a contract between you and the insurance company. It is your responsibility to know your coverage.
NO INSURANCE COMPANY COVERS ALL DENTAL COSTS. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge.
It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid for by your insurance company. We do estimate the patient's portion due in accordance with the information received by your insurance company. Your estimated patient portion is due at the time of service. Please be aware this is only an estimate and an amount may be due after the insurance payment. I also understand that I am responsible for charges not paid by my insurance company and will be responsible for all costs of collections including, but not limited to interest, rebilling fees, court costs, attorney fees and collection agency costs.
